COVID-19 Vaccine

In accordance with New York State law, all members of our workforce, including MMC Holding, members of the voluntary medical and ancillary staffs, and volunteers, students, contractors, vendors, and all other individuals who engage in work on Medical Center premises, are required to be vaccinated against COVID-19.

 

All members of our workforce are encouraged to receive a booster dose of the COVID-19 vaccine as soon as they are eligible. Individuals who are immunocompromised may be eligible for a third dose of the vaccine per CDC recommendations. Appointment scheduling is available online at https://vax4nyc.nyc.gov/patient/s/ or by phone at 877-829-4692.

 

Booster Eligibility Criteria:

  1. Received two doses of the Pfizer or Moderna vaccine at least five months ago
  2. Received single dose of the Janssen/J&J at least two months ago

Personal Protective Equipment PPE Requests

Hospital

The Materials Management department will be restocking all inpatient units daily. N95 masks will be distributed to nurse managers once per day for supervised distribution. If additional supplies are needed the Material Management hotline (x6498) is available from 8:00AM to 8:00PM, 7 days per week.

Ancillary Services (Engineering, FANS, Transport)

Ancillary departments should email their weekly PPE requirement to [email protected]. Supplies will be delivered to the departments.

Off-Campus Locations

Off-Site Departments should place orders for PPE requirements to [email protected] by Tuesdays. Deliveries are made every Thursday.

    Employee Health Services

    Maimonides Medical Center is continuing to require that all employees inform Employee Health Services (EHS) of relevant travel plans – both international and domestic. A form has been created for you to submit your travel plans. This form can be accessed internally through the intranet homepage.

    If you haven’t already, please make a plan to get your flu shot, either through your department or unit or by walking into Employee Health Services.

    Summary

    Vaccinated Staff (or confirmed COVID diagnoses within the past 3 months)
    Unvaccinated Staff
    Domestic Travel (including US territories)
    No testing or quarantine required if asymptomatic. Employees should continue to self-monitor, maintain social distancing, wear the proper PPE required by job function and practice proper hand hygiene.

    International Travel
    Fully vaccinated staff do not need to self-quarantine in the US following international travel. Fully vaccinated staff ARE required to be tested 3-5 days following international travel and continue to self-monitor, maintain social distancing, wear the proper PPE required by job function and practice proper hand hygiene.
    Unvaccinated or partially vaccinated staff are required to self-quarantine for 7 days and get tested 3-5 days following international travel. If test results are negative and the employee is asymptomatic, the employee may return on day 8. In the absence of testing, the self-quarantine period is extended to 10 days. Employees should continue to self-monitor, maintain social distancing, wear the proper PPE required by job function and practice proper hand hygiene.
